When I visited my doctor after getting diagnosed with cancer,he gave me a form to fill in,I filled the few questions that were relevant to me and he filled the rest in,the doctor said it would be more cash for me,I told him I was on attendance allowance and he said its got nothing to do with that,the form is DS1150,anyone know what this is please

Got to the bottom of this,a Macmillan nurse has been assigned to me so I asked her,if one is on the benefit AA,DLA or pip filling that form in goes straight to DWP and if you are not on the top amount of benefit for any of these,it automatically puts you on the top amount without having to fill lengthy forms in.Thanks to all that answered
