Is it true that everyone has to change their old-fashioned driving licence for a new photo version? If so, when does (or did) this come into effect, and what is the penalty if you fail to do so?

No, it's not true,

However you'll get a photocard licence if anything changes, such as your address, or if you renew your licence at age 70.
PS: You can choose to get a photocard licence. (Some people find them advantageous for ID purposes, such as when travelling without a passport on easyJet's domestic flights):

what the little red book thingey ? how old are you ?

there WAS a news article a month or so back that people of my age who did not renew their licences at 65 or 105 or whatever the age was ....
would incur a penalty fine of a few hundred I think

obviously if they were still driving.

and the renewed licence would have to be a new one
