I haven't looked at Chris's links, but swallowing your food before it's been properly chewed up can cause it, as well as taking down air with the larger pieces. The smoking - I don't know.
I don't know what causes Hiccoughs but I do know that eating a teaspoonful of plain white sugar can cure it.
You could be eating too quickly. You could be sat badly when you eat, hence your diaphragm going into spasm. You could have a food intolerance. My sister starts burping EVERY time she has eaten anything with potato in. Have a check and see if it really is every time, or only when you eat certain things. make sure you aren't drinking water etc to wash down food, as this can cause hiccups. Eat slowly, chew properly and see what happens. Then you could try eating meals without wheat or dairy and see what happens. Elimination is the best way to find out if you have an intolerance.

Its actually stopped now, think i was eating to quickly.Ive changed my diet,I used to just eat once a day in evening about 8-9pm,so was probably eating too fast cos I was hungry.
I now eat a sandwich or something at dinner time, and have my tea around 7pm.....no hiccups !!!
