Hope that makes sense.
There are a few things said that insult (maybe that's too strong a word) but are not meant to.

An example, recently a friend done me a big favour and when i thanked him he said "That's ok i would do it for anyone".

I know he meant well but i read it as the fact he would do it for anyone and not just me meant "you're not special to me, im just a nice guy".

I know he didnt mean that.

What other phrases can you think of?

One word - cheers, instead of thank you.
My bad.

Euch!
Have a nice day.
Very American.
Enjoy!
"You're fired! You have 15 minutes to pack your personal belongings from your office, then you will be escorted from the building."
"I don't mean to be rude but...." Well if you think it might be being rude why say it?
"you look pretty when you take your glasses off."

(Correct response: "When I take my glasses off you look pretty.")
Very little offends me. I do get a little annoyed by "with the greatest of respect", because it means the direct opposite generally. This is generally worse from someone who is no expert in your area and has discovered this via Google.

But I take the view that people may attempt to offend me or speak with inadvertence. How they behave is not my issue; how I respond to it is.
ooo - very judicious Barmaid

you must be just an eentsy bit put out when they ask - and what do you do full time ?

I have said: do I look that crap
and got the answer - yes you look half dead
but that is just a reminder that I havent aged well
// I do get a little annoyed by "with the greatest of respect", because it means the direct opposite generally.//

very amusing vignetter in Rick Jolly's Red and Green Life Machine ( Falklands War) - when Ross Adderley a comtemporary of mine says that to an Admiral of the Fleet - and what he says about the opposite meaning
//you must be just an eentsy bit put out when they ask - and what do you do full time ?// I always believe that if people have to ask such a question it is not because they want an answer, but they want a reaction.

The one I am regularly asked is "how do you sleep at night?" - my response varies from "very well, thank you" to "with a cat on my chest" or "naked, apart from pearls".
give 'em hell Barmaid,
and whilst I am here, thanks for all the good advice you have given over the years on equity
When the wife says,"Does my bum look big in this", you reply,"Not as big as in the one you wore yesterday.
But she means well.......said by ex-husband about his mother....far too many times.
