If you are a US citizen I believe not, currently, if you travel by land - but that is changing soon, and already has if you travel by air.

You'll need an ID AND proof of citizenship.

Yes, definitely, even if you are just crossing the border, you need to carry travel documents with you at all times.

To go over into Mexico no, but if you want to come back into the US, very much yes!
