What is meant by this when referring to a man or woman? Are they tall or are they hot?

I wonder whether this is a variant of that other saying: "as...as a yard of pump water". Various words - such as useless, thin, straight etc - may be used to replace the dots. If so, I'd suggest that the man/woman is just being described as tall and thin.
a tall drink of water....something that is hard to swallow? being someone who is difficult take or a task just a little bit too difficult to undertake
I've always heard it to simply describe a tall person.
I believe it is referring to the shaft of a gentlemans privates.
