A friend of mine has been told he may or may not need an operation depending on the results of a scan which will happen in the next couple of weeks. Problem is he has just booked a holiday, and if he needs the operation will not be able to go on the holiday

If he was to take out travel insurance today and declared his condition, and then was told in a couple of weeks that he can't go, would the insurance pay out?

It is an ongoing condition for the past 10 years or so, rather than something that has just happened.
I know it's a bit of a scam but the flights alone cost nearly �1000 and the operation will be over �2000 plus 6 weeks of lost wages for the time off

Just found out he has insurance! But if anyone wants to, they can answer it hypothetically!
But do the insurance know about the ongoing condition? If they do, they may well have excluded claims in relation to it. If they don't, they may well investigate the medical history before paying out. Just because someone has insurance, does not mean they are always covered. The company may have a requirement that they must be told of any impending in-patient treatment. I had to do this with my mother and was told I must cancel now even before anything was certain.
If the insurance company knows about and accepts the ongoing condition then yes, he would be covered. But it's also possible if they know about it they may decide to exclude it from the insurance - ie everything else is covered bar claims arising from the condition. He'd be told if this was the case.
