It is possible to do it yourself, but don't - you've paid the solicitors bill presumably so you've paid her for something she hasn't done. Its a simple form we are talking about - about 30 minutes work.
Get on the phone and complain. Put it in writing and give her 2 weeks to sort it. Then ask who the Partner in charge of professional stds is in the practice and wrote complaining to him/her. Then threaten to involve the Law Society (it won't get that far).
I also had a terrible solicitor and found out they hadn't registered teh property with the Land Registry nearly 3 years on (I only found out after I tried to re-mortgage). My mortgage company were livid with my solicitors and wrote a very stern letter to them which resulted in me being resgieterd within 2 weeks. Maybe your mortgage lender will complain for you?
