Supposedly God. Though there is strong evidence to suggest that he contacted his agent and got a ghostwriter in to do it.

Difficult to say. Obviously the stories were passed down orally in the first instance. Then they had to wait for someone to learn to write.
A variety of people throughout various periods of time.
