Aquarium filter question-Aquaball

I recently purchased an Eheim Aquaball filter. Unfortunately, the instructions are not all that clear. There is a diffuser on the top of the filter which we have above the water level but on the box one of the photographs shows it beneath the water line.

Anyone know what the correct installation?
