they claim 505 in the last 10 years is rare. That is not what I would call a rare. I think it is appalling what do others think.

so those hundreds are over 10 years. I would want to know how early they are released, not just the worst cases, and how early they are released, again not just the worst case. I would also want to know what percentage that figure of 505 is of the total number of prisoners in the system. No system is ever going to be perfect.
Lincoln, the example used, is not a High Security prison but a local prison. The article does not differentiate between the Public and Private sector prisons. Private sectors prisons have been around for about 10 years is that a coincidence. The Prison Service has been starved of much needed investment for many years so the cuts are biting now with many experienced staff leaving early.

Depends what the error is

like if it is a day early - still an early releast error - then I would say it doesnt matter,
or if it is 3pm east gate and he was released 12 midday North gate ( still a release error ) again I would say so what.....
It is a spectacularly poor piece of reporting which as Woofgang and PeterPedant have pointed lacks context. It is impossible to draw any informed conclusions from the article.
