OK Some of you may remember my admission if a dark and macabre crime:

Where I rival the great despots of history for pure evil. Anyway I was up before those lovely people on my local bench this week. Reffering of course to predictions and perhaps even opinions of what should be the penalty for the most heinous of crimes, who would like to guess what actual punishment their worships dolled out? I would offer a prize for the first correct answer but there appears to be no mechanism for it here so I will offer my sincere congratulations to the winner. A clue, costs where £50.

If it was worked out on income - expendeture, which is on the form but clearly ignored, they'd be giving me money! So I have no idea what the purpose of that form is.
I'll stick by my original answer but qualify it a bit. I assume you pleaded guilty so I'll say two thirds of a week's net income (or £250 if you did not give the court details of your income), plus £15 Victim Surcharge, plus the costs you mentioned and six penalty points.

On that basis judge they owe me £50! I don't think the form has any bearing on it. With hindsight I shouldn't have bothered filling it in

I've always thought public floggings would be quite a good deterrent for the sort of person who speeds. They wouldn't deter people like burglars who commit their crimes in secret and hope they won't get caught, but those who misbehave in public could be punished in public.

I had.........none(technically), just a single 'butterfly stitch'.
