changing names on deeds

My husband and I have lived in our house 15years and the mortgage was taken out in his sole name as I hadn't sold my house at the time and was told that my name couldn't be on themortgage as well. There is only a couple of years left on the mortgage but we were thinking of putting my name onto the deeds. Is it possible to do this and how would we go about it??
