One of my drain manhole covers on the driveway has started to sink due to the surround rusting and concrete breaking. Is it covered by the utility company, insurance or by my bank account to fix??!!?? Thanks

I would be extremely surprised if it is covered by insurance. The problem is lack of maintenance and neglect.
its wear and tear should be covered by building insurance, can I ask who you're with?
