Why do Doctors charge so much to write one's medical information on a simple form and then sign it ?

One can get most celebrities autographs for less.

What is the going rate, and why does this not come under the NHS?

because the things that are for the nhs/welfare system are free Eg health related benefits, blue parking badges... if you need certificates for private things then why shouldnt you pay? You are paying a professional for their time and their professional endorsement.
I agree entirely with bednobs.
Otherwise you could get it signed by your butcher...which of course you can't
A doctor, as with all professionals acting in their professional capaciity, can be sued for negligence if they make an error.

Of course they should charge.
