If the pain is muscular I would go with heat.
Try Ibuleve, or the generic equivalent.
I'm sorry, but your username has put weird images in my head of what his injury might be.
Lol
Paracetamol has little effect on back pain, he needs ibuprofen. He needs to find out the cause of the pain and should go to a good chiropractor as GP's are useless with back pain. I wasted 2 years with chronic back pain being told to 'rest' . I went to a chiropractor who gave me exercises to strengthen my core muscles and acupuncture.-touch wood been ok now for over 8 years.
Doesn't matter a "jot" whether you use a hot water bottle or a frozen bag of peas to apply to his back....whichever is more "reassuring."

I agree that ibuprofen is the drug of choice, but there may well be a reason for your GP steering clear of it.

Keep him moving ...the husband that is.

Try Volotrol rub in gel. It's Diclofenic in a gel. I find it very effective. Can get it from Chemist or Supermarket.
I find cold is much better than heat on acute back pain but less comforting!
What helps my backache is having a very hot bath with Radox {muscular} bath salts.
