If a young adult fell soon after an appendix operation could they rupture the stitches they had during the procedure? If that did happen would they need urgent attention to set things right?

yes the skin sutures could come out

would his guts and liver come out thro the hole ?

No - one muscle incision is like one arm of an X
the second muscle incision is ike the other arm of the X

and as you can see they fall together when you take the instruments out

the answer assumes a standard open appendicectomy
Possible....but I doubt it very much......there are 3 layers for closing the wound which would only be about 2" long.
