my cat is now 15 years old and getting very thin, i feed him a can a day, leave dry food down, a bit of cream and milk and worm him regularly, is there anything else i can do?

Just love him to bits in the last few years of his life - as you obviously do, and if he becomes too ill to carry on, have enough love to let him go peacefully.
Yes - put water down instead of the cream and milk - or at least WITH it. One of my cats developed a terrible illness. To make me feel even worse, the vet said that feeding him the rich food which he'd loved so much - cream, smoked salmon, etc!! - had contributed to what happened. In the normal way, cats wouldn't have access to milk after weaning, so it's really not the best thing you can give them. Then, as Andy said - just give him lots of love. x
Have you taken him to a vet? Old cats often develop thyroid problems which cause them to lose weight. It can be controlled by medication but untreated it can cause other problems. He may also have trouble with his teeth. You should perhaps get him checked over. And I agree with ice.maiden - older cats don't do well with rich food and water is better than milk or cream.
you could give him vitamins, i give mine sa-37 its quite expensive but i have found it gave mine a good boost, also i would strongly suggest you take your cat to the vets just in case there is an underlying condition. Quick weightloss is a serious sign of something not right
My cat was the same and the vet said it could either be thyroid or diabetes, he was eating loads as well. I would take him to get things checked out
