About a year ago, my son typed an item into the Google search box, and ever since then, whenever I go to Google, this item appears, so I select it and type in the new item. How can I clear it permanently?

is this the google search box in your browser or on Google's homepage?

If it's in your browser you should be able to right-click the search box and then select "clear search history" or some such.

What browser are you using?

I am not sure I fully understand the issue. Google uses predictive text to start offering search suggestions once you start typing. Does your son's search term always appear or only when you start a search that begins with the same letter?

Google Chrome is the browser. The search term appears immediately I go to Google's home page, before I start typing anything.
try going tools (the little wrench icon), "under the hood" and then clear browsing data. you may want to uncheck the "clear cookies" box but be sure to empty the cache and clear saved form data. Choose "clear data from this period: everything"

if that doesn't work, then clear everything (all boxes checked) for period "everything".

IF google chrome is your browser then you can easily do it by deleting search history from history tab in right hand side top gear icon
