Questionnaire for Assessing People's Computer Skills

Members of my club are asking for help and support with computer issues. Before attempting to address these requests we want to assess the need and find out who amongst our members are competent enough to provide help.

We would like to compile a skills questionnaire but, rather than reinventing the wheel, we are looking for sample questionnaire which we can adapt.

The issues range from the very basic such as how to prepare and send an email and/or attach a photo or file to an email, to the more technical issues of faulty broadband connections and/or upgrading an old computer.

Helpful answers would be very much appreciated.
