Mnemonic

Hi. This is the closest section which I can find to put the question so apologies! I am trying to put a mnemonic together for the word 'REPORT'. It is around reporting accidents and incidents, so for example the first R could be for Ring the emergency services, the E could be Ensure the seen is safe and secure etc. Any help would be gratefully received. Thanks ABers.

I'm sure someone will produce a much better example, but here's an initial stab at it...
R - Rate the gravity of the situation
E - Emergency services required?
P - Personal injuries involved?
O - Offer first aid
R - Record the details
T - Take action to implement procedural changes required if any.

what sort of setting, gadgetgeek? If it were in a dangerous situation the R could be Run.....
