Change of name and CRB check

Hi

I've recently got married and took my husbands surname. About 7 years ago I was arrested and cautioned for shoplifting. My question is will this caution come up on Enhanced CRB check if I changed surname? I am applying for NHS job. Who will be filling the CRB disclosure form? Does it asks you for any previous names??

it will quite possibly show up and yes, you are asked for previous surnames
