It mightn't be the craziest but the name has always made me wonder  : "The Case is Altered".  Used to be at Willesden, I think.
The Foot Hospital" in Aintree

well done neath girl your suggestion will be a hard one to beat.
Playing 'pub cricket' with Mr Spudqueen one day, I asked if it was possible to score a single run, then we went past the Foot Hospital on Scotland Road, Liverpool (not Aintree crisgal)! Pity it's no longer there. Just up the road were two pubs opposite each other, both called the Hamlet!
In Rio de Janeiro there used to be a Queen's Arms... and a Queen's Head... and a Queen's Legs too; not sure when they opened.
There's one in my home town of Dawlish called The Ferret & Radiator. It's name, I believe, was chosen by sticking a pin in the 'phone directory a couple of times.

Slug and Lettuce still exist.  Rat and Carrot was in the 90s - (perhaps 80s too, but definitely in the 90s).

I don't think I'll beat the other suggestions, but I would like to go to the Ferret and Radiator!

PS Your question mark and full stop are the wrong way round - they need to be swapped.

the hairy lemon, i think its in north shields

Pack 0 Cards. combe martin n.devon was built after a local chap george ley won a load of money playing cards   pub

In typical Black Country style, the locals call the pub something different to its' real name. Actually called the Glynne Arms but known as the Crooked House for obvious reasons. Years of mining subsidence have created a building that you feel drunk in before you've touched a drop. A pub where ball bearings run uphill ! its at Coppice Mill, Himley, Dudley, South Staffs

Former locals of mine were The Tiger, The Pennycomequick, The Blue Monkey, The Who'd Have Thought It & The Leg O'Mutton

"The Happy Cheese" in Ashurst, just outside Southampton. Bit strange.
The Illicit Still in Aberdeen is a good one, pity about the smell of fried food though.
Don't know if it's normal, but where I used to live, whenever there was a pub called the Black Swan, locals always used to call it the Mucky Duck.
The Hole in the Wall, i think it's near Borough station in London
There's a pub in Manchester called 'Land O' Cakes', which I always think is a very odd name.

The Poosy Nancies

Who'd a Thowt It

Spinner & Bergamot

Donkey on Fire

The Naughty Stocking - now that's a good one!
