Best going via your GP. Unlike enlargement it's sometimes possible to get it on the NHS if you can prove it's affecting your mental or physical health. One of my friends had one on the NHS when she was 24 - she's a nurse and had terrible backache, not to mention them getting in the way when she was trying to lift patients! It's more complicated than an enlargement so really best going via the NHS initially either way to find out what choices you have available.
