I read this about 10 years ago, and thought it was by Iain Banks, but it seems I am wrong.

It is about a young man who is in a coma, though he is not aware of this till the end of the book. He is living through what happened before being hospitalised. The end of the story is a girl being in the hospital room, confronting him about an attack he took part in against her.

Thanks and here's hoping

Marabou Stork Nightmares by Irvine Welsh I think.
