I know what you are after but I must say I haven't seen one for years. Only thing I can think of is that Jean Greenhowe who has patterns for all manner of novelty things, actually has a chimney sweep pattern. Its around 5" in height if I remember correctly. So, if you know anyone that could make you one maybe that would be an option? I think I still have a pattern so get back to me if you would like to have it.
Have you looked in yellow pages? There are still plenty of chimney sweeps around in my area (Lancashire), though the one I use does not do weddings but some of the others may do.
if in doubt, buy a brush and gets somoene to dress up! Check the yellow pages or ask older locals
Richard Baxter is in Sedgley, which isn't far from Derbyshire. He offers the service you're looking for:

Don't seem to be any advertising do there? Might be a good idea to ask someone at the place where you're having the wedding, they just might know someone.
Not knowing whereabouts in Derbyshire you are - it's a big area (I live about 3 miles out of Derby) I don't know whether this one would be any help
