It forms a part of the personal independence payment https://www.gov.uk/pip/what-youll-get
as woofgang has indicated, you need to apply for PIP (how old are you though?) and if you receive the higher rate for mobility, you will be able to swap it for a motability car. If you are over the PIP age, or need it sooner, you will need to pay for adaptions yourself - go to a garage that does motability vehicles (most of them do) and start your enquiry there
To add further you may need to be re-assesed if your disability is is changing. To get support you would need to be in receipt of the ehanced rate of the mobility component of the PIP payment.
The higher rate of the mobility component of Disabled Living Allowance .. DLA
Or in receipt of War Pensioners Mobility Supplement..
WPMS.
