changed my name over 6 years ago do i still have to declare it to credit file etc if asked

Women who change their name to their husbands on marriage have to declare their nee name all their life, so I imagine it's the same.
The question usually asked on applications forms is ' Have you changed your name in the last 3 years?' Seeing as you changed your name 6 years ago i would stop.
