Mislaid

I have mislaid an appointment letter - cannot find it anywhere. Thank God remember the clinic - have just googled the tele number - when I ring on Monday - you know they ask you for a hospital number etc - I haven't got it - will they be able to source it the appointment with my name and address only. I have only two places I could have put it but it's not there -
