How do I convert an MP3 file to a midi file? Thanks!

You can't.  You'd need to get the sheet music for the MP3 and create the midi file yourself.

I don't know what the evaluation limitations are though, or the cost.
I agreed with OBonio on this one, so when I saw Fern57's post I was amazed. I downloaded the trial version of this software and have to say the result was a bit of a joke. The converted file was JUST recognizable and sounded like it was being played by a drunk pianist.
Ah, but it still could be done (even if the result wasn't any use whatsoever), which is what abrakazabra asked. Now I'm going to have to try it to hear what a 'drunk pianist' sounds like. (o:
