If I buy something from America (I am in the UK) and pay by paypal, will paypal charge for converting the money into dollars?

No, but you will only be offered paypal's exchange rate which may or may not be favourable. You can I think (I've not tried it) have multiple currencies in your account which would be the best way of doing it.

I bought a CD from USA for �19.99. It turned out it was out of stock, and I was refuded only �18.36!! I guess this is because of the exchange rate.

I only have �2 in the account, the transaction would be around �15 in total, but paypal have details of my debit card. Will they take what they need automatically from my debit card?
