I have a very good friend of mine with metastatic pancreatic cancer diagnosed 15 months ago, been relatively stable on meds allowing him to eat he has been having chemo which is now palliative as originally a 4cm tumour on head of pancreas metastatic to liver and in September to the lungs. Last scan showed slight reduction in size so continuing chemo. However he was due to have chemo session today and was told no his haemoglobin levels were too low instead he was given a blood transfusion. His wife is really worried now as he has been extremely breathless the last couple of weeks. I'm just wondering if this maybe the beginning of the end now.

Breathlessness and low haemogloban is common .

I don't know if this helps, but every 2/3 months my Hb drops to under 60 and that means I have to have transfusions. When it happens I can barely walk half a dozen steps I get so breathless. I have the transfusions then I am ok within a day or so until it drops again.
Maybe your friend will also pick up a little after the transfusions.

No it doesnt

you may even get a transfusion to ALLOW them to give chemo.

how  long will he last - honestly none of us know
