What is the meaning and origin of the phrase to "swear black and blue"?

I think its a mix of two sayings. "Swear till you're blue in the face" meaning having no effect, and "beaten black and blue" meaning covered in bruises. I've never heard "swear black and blue", its probably a bastardisation of the first one.
