If there was only ever one president's wife who ran for presidency, it would be ''the''.
If there has been more than one, it would be ''a''.

But what if he was the only former President whose wife ran for office? Surely you would say 'the' so as to differentiate him from all the other former presidents whose wives didn't run for office.
