Well you live and learn, don't you? I always took it literally - a polite hostess offering the Vicar more tea.
Is it not coined by John Betjeman?
I always thought it was from the Monty Python sketch where the vicar had called in unexpectedly on a family.

To cover up the fact that her slob of a husband had farted in front of the vicar, the hostess immediately asked, "More tea vicar?" to which his shocking reply was, "Yes. And I'll have another piece of that f*****g cake as well!"

I thought it came from the expression when someone belched, it was "More tea Vicar?" to which the response always is "No thanks, I've been stuffing tarts all afternoon"
"more tea vicar?" is a non sequiteur, used as a device to draw attention away innuendous, embarrassing or undesirable subject matter. The inclusion of the vicar pertains to said Monty Python sketch, which itself pertains to the gentile vignette of the parish vicar being politely entertained by a member of his congregation on a sunday afternoon over tea and scones. And hard s*xual deviancy.
