I work 7 AM - 4 PM for five days over seven. I have a 15 minute break in the morning and a 45 minute lunch break.

My employer pays me for forty hours - where have the other five hours gone? (5 days X 9 Hours)

I know most employers don't pay you during your lunch break - but that only accounts for 3.75 hours (5 x 45 mins).

Am I entitled to a PAID break? Should they be paying me for 41.25 hours? (5 X 15 mins)

Here in the states, at least here in New York its the law to be paid for a fifteen minute break.
Your total breaks each day (15 mins + 45 mins) add up to 1 hour, so your actual working time is 8 hours per day. (i.e. 40 hours per week)

The only legal requirement is that, since you work more than 6 hours per day, your employer must provide you with at least one twenty minute break. (Your lunch break means that the law has been complied with). Employers are not obliged to pay their employees for any break periods, so your employer is acting within the law by only paying you for 40 hours.
