i know the answer is to look at my policy but can any one tell me what USUALLY happens - is it counted as a car insurance claim for claims history and if it's back screen, do they replace with a heated one if you had a heated one in the first place?

i don't think any policies count windscreen replacement against NCD - though not sure about rear screen.

Rear will need to be proper repalcement for model, so will have element embedded.
Over the years I've had both relaced and claimed on my insurance. They replace like with like so a rear window would have the heating element in it. It's never affected my no claims discount. Although when renewing my insurance in answer to the question "have you had any claims in the last 5 years?" I forgot to mention it as there'd been no damage to the car other than the chip in the windscreen and the person at the other end of the phone reminded me so obviously it is listed on your insurance history even though it doesn't affect your NCD.
It definitely does not affect your NCD for the 'Windscreen' but you will need to check the wording as a rear screen is not a windscreen and may be seen as not covered by this section
Any decent policy regards ALL window glass as "windscreen cover".
