For children, it used to be the ratio of age of intelligence to actual age - so a 10-year-old as bright as an average 12-year old had an IQ of 120. Nowadays, it is derived from a frequency distribution so, (and these figures aren't exact) if you are brighter than 95% of the population of equivalent age your IQ is said to be 130.
