in mirror scenes in movies how do they avoid the camera coming into a reflection shot?

They shoot from an angle, or above or below the line of sight.
They use vampire camera operators
In Goodfellas, they don't. You can see a the cameraman in a mirror in the back of the wedding scene.
They don't always get it right, there have been quite a few movies where the camera can be seen reflected in a window - Pulp Fiction and Christin eare just two examples.
In Carmen Jones, the glorious Dorothy Dandridge is filmed walking along before a row of shop-fronts and there - reflected in each shop window - is the camera trolley trundling along in step with her!
