on the form to receive an Axa payment, but it is with LLoyds Bank.
It appears to be 16 numbers. Do I just leave this blank ?

It usually asks for either a sort code and bank account number OR a building society roll number. Does it ask for a bank account number anywhere?

Yes I have filled in bank account no and sort code

So just leave the BS bit blank

I am sure I sound thick, but time is of the essence I get paid on the 5th Feb and I want to make sure it is all correct
