It's been a very long time since I've worked on one of those.  There are different kinds on Transits, but is that the double rear wheel version?

I think it's a case of removing the brake calipers and then removing the wheel hub with brake disc attached.  I think there are about 6 or 8 bolts holding the disc onto the hub.  Remove them and then tap/hammer the disc off.  Ensure all surfaces are clean and then fit the new disc.

Does that sound about right?  Could you confirm what type it is and if my discription matches how your vehicle is?
