it depends where you live. I used to work for a charity called iain rennie hospice at home
There's the Iain Rennie Hospice at Home - but they are based mostly in the Chilterns..

It can be used as a general term for the support given to people who want to die at home. It is usually operated by the same people who operate your local palliative care. Staff visit at home instead of the person going into a hospice. The care is usually managed by a specialist consultant and support team, the liaison person is usually a nurse, the may give the care themselves or work in partnership with local district nurses and rehab teams.

Its a bit confusing IMO. Its like the term Macmillan Nurses. The term get used for nurses who do palliative care but who are not employed by Macmillan.
