I wouldnt worry about this just yet. You need to provide a MATB1 first anyway. If you are keen to get the letter out of the way you just say when your baby is due when you would like to start maternity leave and if you are taking any annual leave before you go. Depending on where your working try to speak woth someone in Human Resources that can maybe give you a template or a list of things they expect in the letter as I imagine companies vary.
Helloooo yeah you have to get ur MATB1 form - u get it off ur midwife (i cant get mine till im 26 weeks) which is next week. Ask ur midwife - there great for advice.

You can give them a doctor's note saying that it is advisable for you to have a maternity leave before your due date.
