The 1st letter of 5D is the 2nd letter of 4A.
The 1st letter of 6D is the 5th letter of 4A.

Could 4A be RURAL? But why?
Could 5D be CAN? is there a card game called CAN?

Also:
19A It's poetically back to take the chair. (3) S?T
Is 19A SIT or SAT? Why?
18D Rash climbing to conclusions. (5) ?T??S
Is it STEPS? What's the connection with 'rash'?
The T in 18D is the T in 19A.

writer turns up=" pen" backwards. "cut off"= hew. And the relative part is the nephew.

I suppose second one could refer to the n from one, but that still leaves spai to find...

Thanks Gazzawazza. Maybe that's all it was, just a country with letter 'i' in it.
maybe the five refers to a hand (span) plus a one(i). Not sure though
19a could actually be TIS, which would make 18D - S - - S.
Could 18d be STOPS? That is SPOTS (rash) backwards

Nut waht's the connection between STOPS and 'climbing'?
As it is a 'down' clue, I think 'climbing' means the word goes in backwards
