If a fit and healthy person suddenly finds themself being a amputee or loses use of a leg/arm, do they have to retake a driving test in their adapted vehicle or just learn how to use it?

yes, they have to retake their test in their adapted vehicle and are restricted as a driver to that vehicle.  Look on the DSA's website.
