Double vesectomy??

My husband went for one in july this year, all went straight forward. He put his 2 samples in 8 and 10 weeks later, then we got a letter saying that basically they were both still alive with sperm, so could he do 2 more, he did 2 more 2 weeks apart. Then we got a letter saying the surgeon needs to see him about his results. And now because he has put in 4 samples in and they've all been alive he has to go have the op done again. Has anyone ever heard of this before?

did he do enough ejaculations in the 8 week period? he would have to do around 25 to be sterile

However, I do remember the doctor saying that sometimes the tubes can actually grow back and maybe this would be the reason.
