how is it decided where the conversion will be kicked from?

The kick is taken in line with where the ball was grounded. If this is near the touch line, the kicker will move the tee further away from the try line to take the kick, so it improves their angle onto the posts. If a try is scored under the posts, the kick is taken only about 10 yards out..

very informative :)
I remember our maths teacher who once tried to tell a not very interested class a formula that would give you the shortest distance plus the best angle. Can't remember what it was, though.
