Has my doctor breached the DPA

My company requested a medical report from my doctor and I requested that I see the report before its sent to my employer, the report went to my employer without me seeing it first, also after seeing the report from my Manager, the doctor mentioned a disapline and grievence I had in a previous company, but I when I started with my new employer I stated the reason I left was I resigned, (I left my previous employer with a compromise package where the reason for leaving was resignation on any reference details). Also dates and data in the report is incorrect, Is this a breach of the DPA? and what can I do about it if it is?
