The online form is necessary otherwise they do not let you on the plane. There is no guarantee that they will allow you entry at the other end. There is no right of appeal if rejected.
There are some dodgy sites that try and charge you - the ESTA is free - make sure you use the right site here https://esta.cbp.dhs.gov/
BIG TIP - you do not get an email confirmation so when you get to the final screen with your acceptance on it take a screen grab of this and print it out and keep this on you when you check in at the airport.
Auntie Bertie, I did this but my printer fouled up. When I went back to reaccess the site, I still got the page that said I wasapproved but it just showed my application number but no name. I printed this out but wonder if there is any way I can print something with my name on?
Have you tried going back in to Edit your info - and see if you can get to that final screen with your conf # and your name and details - not sure if this will work but just an idea!
