They have to be trained - but hopefully their mum would have seen to this before being separated. The new owners just have to train the kitten where the litter tray or cat flap is. That is my experience anyway. You are bound to get a few accidents in the early days.
I have had three kittens from the age of 8weeks and up and once you have shown them where the littler tray is they usually know how to use it, otherwise I would pick up the poop or soak the wee if they have an accident and place it in their litter tray so they know what to do with it.

If you mean how do they know from birth then I can't help as I have never had a kitten from that young
I have had many cats (and they have had a few kittens in their time) and I always found that mum cleaned the kittens up in the first few weeks herself. When the kittens were a little bit older (about 3 weeks), I used to simply pick them up and pop them in the (clean) cat litter tray - they very soon started to use the tray. They actually really enjoyed the cat flap... what a brilliant toy that was ! In...out...in....out.....in....out... hehe When they worked out that hunting (twigs, leaves and worms) was good fun they used to bring their finds indoors for me too! Not good for me though as I'm not too fond of worms....
